失眠和疫苗效果有关系吗?

近期发表在国际著名医学期刊《柳叶刀》The Lancet的文章指出,睡眠不足可能会影响接种疫苗的保护力。在流感疫苗后的10天,连续4个晚上睡眠限制为4小时的接种者IgG抗体滴度不到正常睡眠接种者IgG抗体滴度的一半。同样乙肝疫苗后,睡眠时间越短,对疫苗产生的抗体反应就越弱。临床研究还是动物模型研究显示,睡眠不好会影响身体机能的变化。“晚上11点到早上8点,特别是晚上一两点是人体精力恢复的最佳时机,如果睡眠得不到保证,身体机能的一些恢复也得不到保证,肯定会影响身体的免疫。所以,接种疫苗期间,如果睡眠不好,老是失眠,从理论上来讲确实有可能会影响疫苗的疗效。”

相关信息:男性,年龄51-60岁之间,体重61 -70kg之间,大便正常,失眠,偶尔喝酒,有吸烟。

近半年一直失眠,每晚2点左右醒后需较长时间再入眠。伴有腋窝下和左胸周肌肉抽紧。做过几次心电图,均显示正常,之前甲状腺结节,但已基本消退,指标也基本正常,血检血脂胆固醇超标,其他正常。

I have had insomnia for nearly half a year, it takes a long time to fall asleep after waking up around 2 o’clock every night. Accompanied by muscle tightening under the armpit and around the left chest. After several electrocardiograms, all results showed normal. 根据患者的描述,应该属于中医所讲的不寐,也就是俗称的失眠。而且从虚实来分辨的话,考虑虚证的失眠可能性大一些,尤其要考虑肝血虚的情况。

中医理论认为,五脏之中肝藏血,血舍魂。一个人如果肝血不足,魂不能好好地在肝血中休息,就会出现睡眠不好的情况。从患者每晚都是两三点钟醒来,而醒来后很难再次入睡的情况来看,这个时间段正好是十二经络中肝经的运行时间,患者应该是肝经血虚,从而出现此时醒来且难以再次入眠的情况。此外,中医理论也认为,肝藏血在体为筋,,肝血不足则经络失养,容易抽筋,这恰好也与患者的情况相符合。因此,这里认为,患者若要改善睡眠,可以多注意养肝血。

由于中医认为肝肾同源。女性七七之后,男性八八之后肝肾都渐亏,因此患者平时的调养可以选择滋补肝肾、养血安神的药膳汤水。考虑患者血脂的问题,要注意不要肥腻。

这里推荐患者可以用熟地20克黄精20炖瘦肉食用。熟地黄精功能滋阴养血、滋补肝肾,炖瘦肉食用既可以养肝血助睡眠,也适合三高人群服用,对血糖、血脂也有一定的调节作用。另外,患者也可以试试用中药党参15克、酸枣仁30克、白芍15克、炙甘草15克、茯神30克、熟地黄15克,煎煮饮用。此方中党参健脾养血、熟地养血滋阴、白芍甘草养肝柔筋、茯神健脾安神、酸枣仁养血安神,全方共同起到养血安神助睡眠的作用。
