叶阿姨，73岁，身高156厘米，体重66公斤。叶阿姨患有高血压、高血脂近20年，一直遵医嘱服西药（施慧达、辛伐他丁），血压、血脂水平基本控制在正常值。半年前心电图检查发现ST波异常，提示心脏供血不好，服用了一段时间的复方丹参滴丸情况改善停药。近半月来因家务劳累，经常觉得心慌心悸伴有胸闷，检查心电图又显示ST波异常和频发室性早搏，心率88次/分，医生加用倍他乐克12.5mg,每日两次服用。请问因目前仍然觉得心慌心悸胸闷，乏力，口干，舌质暗红有瘀斑，舌苔干少，纳眠可，二便正常，中医该如何治疗和处理？
Case
Ms. Ye, 73 years old, is 156cm tall and weighs 66 kg. Ms. Ye has suffered from hypertension and hyperlipidemia for nearly 20 years. She has been taking western medicines (Shihuida, Simvastatin) as prescribed by the doctor, and her blood pressure and blood lipid levels are basically controlled at normal values. Six months ago, an abnormal ST wave was found in the electrocardiogram, indicating that the blood supply to the heart was not good. After taking the compound Danshen Dropping Pill for a period of time, the condition improved, and the drug was discontinued. In the past half month, due to the fatigue of housework, she often felt palpitation and chest tightness. The ECG showed abnormal ST wave and frequent premature ventricular contractions. The heart rate was 88 beats/min. The doctor added Betaloc 12.5mg, twice a day. 这里分析患者的情况：患者叶阿姨既往有高血压、高血脂，一直规范服药控制。半年前心电图检查发现ST波异常，提示心脏供血不好，服用了一段时间的复方丹参滴丸情况改善停药。近半月来因家务劳累，经常觉得心慌心悸伴有胸闷，检查心电图又显示ST波异常和频发室性早搏，心率88次/分，加用倍他乐克12.5mg,每日两次服用后，目前仍然觉得心慌心悸胸闷。就目前患者出现的心悸心慌胸闷的症状来看，中医诊断考虑属于中医内科“心悸”的范畴，从患者兼见的乏力，口干，舌质暗红有瘀点，舌苔干少等症状表现来看，症属于气阴两虚型。因此，建议患者在中医治疗上可以采用益气养阴、活血通脉的方法来治疗。

Judging from the current symptoms in patients, Traditional Chinese Medicine diagnosis is considered to belong to the category of “heart palpitations” in Traditional Chinese Medicine internal medicine, from the symptoms of fatigue, dry mouth, dark red tongue with petechiae, and less dry tongue coating. The disease belongs to the deficiency of both qi and yin. Therefore, it is recommended that patients can use the methods of nourishing qi and nourishing yin, promoting blood circulation, and dredging meridians in Traditional Chinese Medicine treatment.
具体用药方面，建议患者不妨可以试用稳心颗粒。中成药稳心颗粒组方借鉴古方“心动悸、脉结代，炙甘草汤主之”的宗旨，选用中药黄芪、黄精、三七、琥珀、甘松等成分，具有益气养阴、活血复脉，调节机体气血、稳定心悸的作用。临床上中成药稳心颗粒适用于气阴两虚、心脉瘀阻所导致的心悸不宁，气短乏力，胸闷胸痛的患者，对于室性早搏、房性早搏症见上诉症状的患者均适用。因此，对于患者叶阿姨来说，在服用原有降压降脂药物的同时，如果出现了上诉症状，也是非常适用的。患者不妨在专业医师的指导下选用。当然，对于患者目前的症状表现，也可以采用中药汤剂进行治疗，比如可以试试太子参30克、白芍15克、炙甘草15克、丹参15克、降香10克（后下）瓜蒌皮10克，上诉药物水煎服，每日一剂，餐后服用。当然，这里由于不是面诊，用药建议仅供参考，具体用药需要遵医嘱。

Clinically, Chinese Patent Medicine Wenxin Granules is suitable for patients with palpitations, restlessness, shortness of breath, fatigue, chest tightness and chest pain caused by deficiency of both qi and yin and blood stasis caused by blood stasis. It is suitable for patients with premature ventricular contractions and premature atrial contractions. Therefore, for the patient, Ms. Ye, it is also very applicable if the symptoms of appeal appear while taking the original antihypertensive and lipid-lowering drugs. Patients may wish to choose under the guidance of a professional physician. Of course, for the patient’s current symptoms, Chinese herbal decoction can also be used for treatment, such as 30g of Taizishen, 15g of white peony root, 15g of Zhigancao, 15g of Salvia miltiorrhiza, 10g of delicacy, and 10g of melon, 10g of Loupi, decocted in water, one dose a day, after meals.
